Join us for an important and informative workshop on Advanced Care Planning which is the process of determining your future medical care if you are incapable of speaking for yourself.

Do you have an advanced directive? If not, do you know why you might need one?

- The components of an advanced directive, which includes a Living Will and the appointment of a health care agent

- The importance of appointing a health care agent and the types of decisions they can make for you

- Available tools and resources, both paper and electronic, to help you complete an advance directive

- Tips on starting conversations about your end-of-life wishes with loved ones so you can successfully complete your advance directive


Presented by CaringMatters, a nonprofit providing bereavement support services. In addition to community education programs, CaringMatters provides social and emotional support free of charge to individuals in Montgomery County facing serious illness or grieving the loss of a loved one.
